I understand, acknowledge and agree to the following:

1. That there will not be any physical examination involved and the consultation services shall be provided remotely. The opinion delivered by the doctor / physician shall solely be based on the verbal communication between the concerned doctor or the physician and the patient and the test reports and other information provided / uploaded by me. Therefore, I understand that I am responsible to furnish my illness in the greatest detail for the doctor to advise the appropriate advice / prescription.
2. That EVA HEALTHPLACE Services are not meant in any way for emergency and life-threatening conditions and if such event occurs, it is advised to take the patient to the nearest hospital.
3. User discretion is advised while submitting/uploading the required personal and clinical information on the specified EMR platform.
4. This EMR information shall include my contact details, medical history, Investigation results/reports, prescriptions, and other relevant details.
5. That if the information provided by me is false/deceptive/inaccurate or misleading, EVA HEALTHPLACE reserves the right to cancel my appointment without assuming any liability whatsoever. Hence, it is advised to submit/upload all relevant and accurate information.
6. That the information provided by me may be used by EVA HEALTHPLACE for the purpose of analysis, research, training and disclosure (where required) to its affiliates, agents and government authorities.
7. That the information provided by me can be kept with EVA HEALTHPLACE indefinitely and can be used without revealing my identity.
8. EVA HEALTHPLACE or the consulting Doctor/Medical Practitioner shall not be liable for misdiagnosis / faulty judgment / interpretation error / perception error/Adverse events/ inefficacy of prescribed treatment or advice, validity of the advice or prescription provided by the consulting Doctor in my country of residence & unavailability of the recommended or prescribed treatment or medication under any condition or circumstances.
9. That any prescriptions that you acquire from a Doctor shall be solely for your personal use. You agree to fully and carefully read all provided product information and labels and to contact a physician or pharmacist if you have any questions regarding the prescription.
10. You understand your Doctor, in his or her sole discretion and professional judgment, may determine that telemedicine services are not appropriate for some or all of your treatment needs and, accordingly, may elect not to provide telemedicine services to you through the EVA HEALTHPLACE platform.

After understanding the contents of the above and after clarifying all my doubts, I have signed this General consent & the Disclaimer to execute my informed choice. At any point of time, now and / or in future, I will not hold and / or caused to be held the concerned health professional(s), staff of EVA HEALTHPLACE Personnel responsible / liable for not achieving the expected outcome(s) of the proposed “medical consultation or any further advise” and / or for achieving undesired complications and I do hereby relieve them of all such liabilities.
